Abstract

The human’s brain is still the most amazing neural network in this age of machine learning and yet we still make amazingly scary mistakes, systematically and by being human. We design sophisticated algorithms to screen and catch errors while we also absentmindedly ignore the many pop ups that snake through the Swiss cheese model of medication errors which resulted in harming the patients. To err is human and to care is a higher human function. A caring pharmacist is one of the last defenders against the holes in the medication journey to deliver safer care to our patients.
